Description ofthe thing to beimported

Submit a separate application form for each foreign supplier (i.e.,exporter) and each destination within Canada. You must obtain animport permit prior to the material being exported to Canada.

The importer must be a Canadian citizen or permanent resident, asthose terms are defined in subsection 2(1) of the Immigration Act; or aperson who is authorized under the laws of Canada to reside inCanada for a period of six months or more and will have thepossession, care or control of the thing at the time of importation; or aCanadian registered corporation with a place of business in Canada.

The complete name, address, telephone number and if applicable thefacsimile number of the importer must be provided. Furthermore, thecomplete name and address of the importer must be the same as thatappearing on all invoices and Canada Customs import documents.

The place (provide complete address details) within Canada where thematerial can be inspected prior to distribution or disposal.

The name of the Canadian border crossing or the Canadian Port (e.g.,harbour, airport) through which the material will enter Canada.

How is the material going to be conveyed / brought into Canada, by rail,truck, private vehicle, aeroplane or ship.

The complete name, address, telephone number and if applicable thefacsimile number of the exporter (i.e., the name of the person /company in the foreign country who is exporting the material toCanada) must be provided. The complete name and address of theexporter must be the same as that appearing on all invoices andCanada Customs import documents.

Name the foreign country and the place (e.g., State, County) within thatcountry where the material was grown, raised, cultured or produced.For example, in the case of pathogens, insects or nematodes, pleaseprovide the foreign country and place (e.g., State, County) where theorganism was originally collected from its native habitat.

The foreign country and place (e.g., railway terminal, harbour or airport)through which the material is being transported directly to Canada.This section must be completed, if the country is different from that ofthe foreign exporter.

Why do you want to import the material? Do you want to import thematerial for commercial or private propagation purposes, humanconsumption, scientific research, educational, commercial processing,industrial or exhibition purposes. If the material is being imported forscientific research purposes, please provide a brief outline/descriptionof your proposed research project.

PLANT MATERIAL

Provide the common and scientific / botanical name (i.e., genus andspecies) of the plant, including the type (i.e., seeds, rooted cuttings,scions, bare rooted plants, plants in soil or other growing media, etc.) ofplant material. If the plant material has been genetically altered /modified, please provide a description of the novel traits and /or DNAinserts.

Description

Le nom complet, l'adresse, le numéro de téléphone et, le cas échéant,le numéro de télécopieur de l'importateur doivent être fournis. De plus,le nom complet et l'adresse de l'importateur doivent être identiquesà ceux qui apparaissent sur toutes les factures et les documentsd'importation de l'Agence des douanes et du revenu du Canada.

Le lieu (donner l'adresse complète) au Canada où le matériel peut êtreinspecté avant sa distribution ou son élimination.

La façon dont le matériel sera transporté ou acheminé au Canada :train, camion, véhicule privé, avion ou bateau.

Le nom complet, l'adresse, le numéro de téléphone et, le cas échéant,le numéro de télécopieur de l'exportateur (c.-à-d. le nom de la personneou de l'entreprise dans le pays étranger qui exporte le matériel auCanada) doivent être fournis. Le nom complet et l'adresse del'exportateur doivent être identiques à ceux qui apparaissent sur toutesles factures et les documents d'importation de l'Agence des douanes etdu revenu du Canada.

Pourquoi désirez-vous importer le matériel? Si le matétiel végétal est importé à des fins scientifiques,veuillez fournir un bref exposé ou une description du projet derecherche envisagé.

MATÉRIEL VÉGÉTAL

Donner le nom commun et scientifique ou botanique (genre et espèce)du végétal, y compris le type du matériel végétal (p. ex. semences,boutures racinées, greffons, plants à racines nues, plants en terre oudans d'autres milieux de culture, etc.). Si le matériel végétal a étégénétiquement modifié, veuillez fournir une description des caractèresnouveaux et/ou les inserts d'ADN.

SOIL AND OTHER THINGS

Please provide a general description of the thing, including otherparticulars such as its composition if applicable.

In the same section of the application form, please provide thequantity of the material to be imported.

Please indicate on the application form, by checking the appropriatebox, whether or not the material intended for importation has beengenetically modified / altered. If the material is being shipped by mailor courier service, please indicate the number of packages to beimported.

Also provide the name of the permit applicant in block letters. Theapplicant must then sign and date the application form.
