Abstract In 2010, 9 439 patients with end-stage renal disease living in 23 regions (Alsace, Aquitaine, Auvergne, Basse-Normandie, Bourgogne, Bretagne, Centre, Champagne-Ardenne, Corse, Franche-Comté, Haute-Normandie, Île-de-France, La Réunion, Languedoc-Roussillon, Limousin, Lorraine, Midi-Pyrénées, Nord-Pas-de-Calais, Pays-de-Loire, Picardie, Poitou-Charentes, Provence-Alpes-Côte-d’Azur et Rhône-Alpes) covering 64 million inhabitants (98% of the French population), started renal repla- cement therapy (dialysis or preemptive graft): median age was 71,0 years; 3,2% had a preemptive graft. The overall crude annual incidence rate of renal replacement therapy for end-stage renal disease was 149 per million population (pmp), with significant differences in sex and age-adjusted incidence across regions (109 to 396 pmh). At initiation, more than one patient out of two had at least one cardiovascular disease and 40% diabetes (94% Type 2 non-insulin-dependent diabetes). On December 31, 2010, 37430 patients living in the 23 regions were on dialysis: median age was 70,9 years. On December 31, 2010, 29,841 patients were living with a func- tioning graft: median age was 54,9 years. In these 23 regions, the overall prevalence of dialysis was 720 pmp, that of renal graft, 470 pmp and the overall rate of renal replacement therapy for end stage renal disease, 1,060 pmp with significant differences in age-adjusted prevalence across regions (841 to 2,745 pmh). In the 2002-2010 cohort of 53436 incident patients, the overall one-year survival rate was 83%, 51% at 5 years. Survival decreased with age, but remained above 50% at 2 years in patients older than 75 at RRT initiation. Among the 9,108 new patients starting dialysis in 2010 in the 23 regions, 6% had a BMI lower than 18,5kg/m 2 and 21% a BMI higher than 30. At initiation, 66% had a haemoglobin value lower than 11g/l and 11% an albumin value lower than 25g/l. The first haemodialysis was started in emergency in 34% of the patients and with a catheter in 54%. On December 31, 2010, 7% treated in the dialysis units of the 23 regions received peritoneal dialysis, of which 42% were treated with automated peritoneal dialysis. 95% of the patients on haemodialysis had 3 sessions per week, with a median dura- tion of 4 hours. In the 2002-2010 cohort of incident patients in 20 regions under 60 years, the probability to be at least once on the waiting list for a renal graft is 62% at 3 years. In 2010, 2,864 patients received a renal graft. On December 31, 2010, 8,180 patients were on the waiting list for a renal graft in the transplantation centers of the 23 regions. 2. À l’initiation du traitement, plus d’un malade sur deux avait au moins une comorbidité cardiovasculaire et 40 % un diabète (diabète type 2 dans 94 % des cas). Au 31 décembre 2010, 37 430 malades résidant dans ces 23 régions étaient en dialyse (âge médian 70,9 ans). inférieur à 11 g/l et 11 % une albuminémie inférieure à 25 g/l. La première hémodialyse a été effectuée en urgence pour 34 % des malades et dans 54 % des cas sur un cathéter.

Au 31 décembre 2010, parmi les 37 320 malades traités dans les structures de dialyse de ces 23 régions, 7 % étaient en dialyse péritonéale (dont 42 % en dialyse péritonéale automatisée). En hémodialyse, 95 % des malades avaient eu 3 séances par semaine, d’une durée médiane de 4 heures.

Dans la cohorte des malades de moins de 60 ans ayant débuté un traitement de suppléance entre 2002 et 2010, dans 20 régions, la probabilité d’être au moins une fois inscrit sur la liste d’attente d’une greffe rénale était de 62 % à 3 ans.

En 2010, 2 864 malades ont été greffés. Au 31 décembre 2010, 8 180 malades étaient en attente de greffe de rein dans les centres de transplantation des 23 régions.

La probabilité de survie diminuait avec l’âge mais restait au-dessus de 50 % à 2 ans chez les malades de plus de 75 ans à l’initiation du traitement de suppléance.
