MALARIA IN THE WORLD One third of

the world populationis affected

Malaria remains the most important widespread parasitic diseaseClinical cases : 270 to 480 millions occured in AfricaMortality : 1,5 to 2,7 millions a year,

Large majority in Africa, childrens aged 0 – 5 years

The major concern in the treatment of the disease :increase resistance of the malarial Plasmodium falciparum to drugs.- In Africa : chloroquine (Nivaquine) resistance 70%- In Asia : mefloquine (Lariam) (important secondary effects) resistance 50%

5

Doubling of the population of countries with endemic

malaria in the past two decades.

Population increase

Loss of interest in malaria programs during the last 25

years after the elimination of malaria in north America

and Europe.

Quasi-absence of new drugs

Global warming, associations with agriculture practice.Environmental and climatic changes

Increasing travel between endemic areas and free areas

in endemic countries.

Travel

Population migrations, collapse of Health services…War and Civil disturbances

Increasing resistance to low cost insecticides

(pyrethroids).

Mosquito resistance to

insecticides

Increasing resistance to low cost treatments

(chloroquine, antifolates).

Parasite resistance to drugs

Factors impacting on malariaFactors impacting on malaria

ClNa

Na+ Cl-

--> comment éviter cette séparation?

47

Malaria and toxoplasmosis infections are caused by related

Apicomplexa protozoa:

Plasmodium falciparum and Toxoplasma gondii.

Recently, more attention to Toxoplasma gondii :

most common cause of focal encephalitis or focal

central nervous system abnormalities

in immunocompromised patients with AIDS or in transplant recipients

pyrimethamine plus sulfadiazine or clindamycin are the drugs of choice for therapy

but some patients are intolerant.

toxicity effects = allergies (especially in AIDS patients)

there is currently a great need for alternative therapies.
