Abstract: At Pharmapack Europe 2012 we saw this year that compliance is a big topic amongst suppliers of packaging solutions for the pharmaceutical industry. Many packaging solutions for this industry now go far further from their primary function of protecting the content, and include features like easy dosage and application, child resistance, senior friendliness and tamper evidence. Electronics is also making its way towards providing intelligent information to patients to improve compliance. The other major trend that we have noticed is the increasing use of lighter and safer polymer materials for applications where glass is traditionally used like for bottles, vials and syringes. *** Résumé: Au salon Pharmapack Europe 2012, nous avons vu cette année que le respect du traitement par les patients est une problématique que les fournisseurs de solutions d'emballages pharmaceutiques intègrent de plus en plus dans leurs offres. Aujourd'hui, certaines solutions d’emballage pour cette industrie vont beaucoup plus loin que leur fonction première de protection du contenu, et intègrent des caractéristiques telles que la facilité de dosage et d'application, la sécurité enfants ou la sécurité anti-effraction. L’électronique fait également son entrée permettant le développement d’emballages intelligents qui aident les patients dans la prise et le respect de leur traitement. L'autre grande tendance que nous avons remarquée est l'utilisation croissante de matériaux polymères légers et plus resistants à la casse pour les applications où le verre est traditionnellement utilisé, comme pour les flacons, ampoules et seringues. INTRODUCTION

This year, VeilleSalon went for the first time to Pharmapack Europe in Paris. This International conference and exhibition is dedicated to innovation in pharmaceutical packaging and drug delivery systems. It is a place where manufacturers and suppliers of medical devices, packaging, and production equipment present their products and innovations. The 2012 edition attracted over 2,800 visitors from medical and pharmaceutical companies, biopharmaceutical and veterinary laboratories. Over 260 international companies presented their latest innovations, products and services for the design and manufacture of packaging and dispensing systems for medicinal products. In this document we report on what VeilleSalon will remember from Pharmapack Europe this year. Glass replacement is a big topic across all industrial sectors as lighter materials are sought in order to reduce the impact on the environment of human activities. Glass has been traditionally used for pharmaceutical packaging and delivery systems as it is inert and transparent. Some plastics are now able to advantageously replace glass offering transparency, inertness, lightness and greater resistance to breakage. These polymers are further optimized to provide suitable properties like gas and moisture barriers. Easy application and easy dosage was another big topic as patient’s compliance is highly dependent on packaging ease of opening and use. At the same time, medical packaging must not be opened too easily and should feature some degree of security in order to prevent children from ingesting drugs. Although Child Resistant & Senior Friendly packaging have had a lot of success across the Atlantic over the past decade, these types of packaging are not yet widely used in France, albeit the child resistant cap that even adults sometimes find difficult to open. The three other packaging exhibitions VeilleSalon went to or will go to this year are: PCD and Aerosol&Dispensing Forum (February), CFIA (March), and Emballage (November). EASE OF USE AND ACCURATE DOSING

Compliance is a major issue within healthcare today as non-adherence to prescribed medications may cause death. Non-adherence can constitute many forms: not having prescriptions filled, taking incorrect dose at incorrect times, forgetting doses or stopping before the recommended time. Non-adherence is one of the most common causes of uncontrolled chronic conditions while adherence, also called compliance, has shown to improve life expectancy by reducing the development of complications. Compliance describes the extent to which patients follow the recommendations of their healthcare provider on medication, diet and lifestyle modification to ensure favorable clinical outcomes. With the current difficulties in the world economy and the subsequent pressure on healthcare costs worldwide, companies are looking into patient compliance as a means of reducing the burden on the healthcare system while improving patients’ health condition. Convenience, ease-of-use, efficiency and accurate dosage are therefore of critical importance for drug delivery, be it for patients themselves or for practitioners.

Coradin is a plastic injection molding company that offers packaging solutions for cosmetics and pharmaceuticals. At Pharmapack 2012, Coradin presented their recently launched eco-responsible packaging range. Ecodis is produced with 100% recyclable raw materials, and the design has been optimized to keep weight to the strict minimum as well as using only as much plastic material as needed. The container, dropper and protection cap are made of polypropylene (PP) and the soft part to fill out the product is made of thermoplastic elastomer (TPE).

With Ecodis, the cosmetic product application is done with new actuation. Indeed, by simply pressing on the supple bottom part of the rigid tube, the cosmetic product is applied onto the skin. Ecodis is currently available in 10ml, 15ml and 30ml sizes. The other original aspect of Ecodis is to propose a 2-in-1 version composed of a cartridge which is simply opened by pressing on the soft bottom part of Ecodis. After shaking, the formulation is reconstituted and is ready for use.

Ecodis has been designed for skin care fluid products, make-up (foundation) or hair care formulations. It can be decorated on request, and is easy to fill, assemble, transport and use. Neutroplas is a Portuguese company that specializes in the production of plastic primary packaging and dosage devices for the healthcare and pharmaceutical markets. This product, which is an evolution of a tried and tested process at Neutroplast, was hallmarked by other companies as it was Neutroplast that first introduced triangular bottles into the Pharmaceutical market.

SLG Pharma GmbH & Co. KG is as a subsidiary of SLG Kunststoff GmbH that was founded in 2009. SLG Pharma develops and produces innovative dosing systems. Their product portfolio includes PickDose, TubeDose and TubeDose Linear. PickDose is a precise measuring pipette with a variably lockable dosing volume. The measuring scale is set to the desired dosing volume by twisting. To pick up precisely the set quantity, PickDose is simply filled by drawing up to the limit.

Safe handling: The innovative sealing system guarantees clean product discharge. Materials: PickDose is manufactured under clean room conditions. All materials have EP and USP approval and can be sterilized. Use: PickDose is indispensable if high-precision requirements have to be fulfilled. The desired strand length is variably adjusted on a numbered scale and can be firmly locked (Patent pending).

The dosing system is manufactured under clean room conditions and is suitable as an application and measuring aid for human and veterinary medical ointments, creams, lotions and gels, for cosmetic products and also technical applications. It saves the manufacturer from having to give vague dosing instructions involving “pea-sized” quantities which have never been made for reliable and safe treatment. Exact volume extraction in three steps:

1. Set the quantity by setting the recommended dose on the quantity scale accordingly. 2. Press the tube to fill the dosing feeder precisely with the set quantity. The user can see this from the dosing arrow, which gradually moves up to the limit on the marker. 3. Remove the product by opening the lid and pushing back the dosing arrow to its initial position. The marker remains in place and only moves at the next application if the therapy requires a different dose.

TubeDose Linear is a new development of TubeDose that was presented of Pharmapack 2012.

TubeDose Linear can deliver from 0.5 to 2 g of product in a very clean and simple manner.

Just like for TubeDose, volume extraction is achieve in 3 steps:

1. Select the quantity by setting the recommended dose on the quantity scale accordingly. 2. Press the tube to fill the dose chamber precisely with the set quantity. In parallel the dosing cap moves upwards. 3. Dispense the product by opening the lid and pushing down the dosing cap to its initial position.

Primequal is a Swiss company specialized in prefilled syringes and injection devices for pharmaceutical drug delivery. Primequal has developed a new type of Dispenser - Doser - Injector. The Preciquant dispenser uses a patented airless propelling system based on a lever. It can be equipped with a specific nozzle that does not allow oxygen to enter the container. The efficiency of the product to dispense is preserved and treatment efficiency is thus increased.

Primequal Preciquant received the International « Reddot design award » in 2007 and Pharmapack Award in 2011. In December 2010, Primequal launched the world’s first automatic dosing system for botulinum toxin. Talent BT is an automatic single button injection device developed to allow a safer, faster and repeatable dose delivery for unit based botulinum toxin injections. Such spray devices can help the patients count their doses and consequently see how many doses remain before they need to refill their prescriptions (e-Dose Counter platform). e-Dose Counter is a cost effective solution which meets regulatory recommendations for nasal and sub-lingual spray delivery of controlled substances. The use of electronics allows the counting display to be large and highly visible making it suitable for patients of all conditions and ages. Electronics can also provide patient comfort features such as acoustic feedback and flashing displays to inform and warn.

In addition electronics can allow self dosing of treatments such as breakthrough pain medication without direct healthcare professional supervision and can protect the patient with lockout systems that would prevent accidental overdosing and diversion with potent controlled substances for example.

e-Lockout counts and displays the number of actuations and also prevents the device from being used for a period of time after a pre-defined number of actuations. Its locking system which warns of overdosing is recommended by regulators for nasal and sub-lingual spray delivery of controlled substances used in breakthrough pain management. In addition to counting and displaying the number of actuations and locking, some of the key features which can also be incorporated into e-Lockout include patient aids and feedback as well as data transmission.

CHILD RESISTANCE AND PRODUCT PROTECTION

According to the World Health Organization (WHO), in the European region, the third-leading cause of unintentional death in children through age 14 is acute poisoning. WHO estimates that 3,000 children in that age group die each year, and that 90% of those poisonings take place in the home. In its World Report on Child Injury Prevention, published in 2008 The WHO said that “Child resistant packaging is one of the best-documented successes in preventing the unintentional poisoning of children.” Data were cited for a number of affluent nations. In England and Wales for example, according to the WHO, unintentional poisoning deaths amongst children under the age of 10, fell from 151 per 100,000 in 1968 to 23 per 100,000 in 2000. Reliable child-proof packaging must however combine two contradictory needs: safety against unauthorized access by children and user-friendliness, especially for senior citizens. Child-resistant packaging is supposed to make drug access more difficult for children whereas senior-friendly packaging should facilitate the intake of medicines. The other feature packages for drugs must incorporate is product protection against moisture and gaz. This is usually achieved through a desiccant that prevents the medicine to degrade following the opening of the bottle. Desiccants may however saturate and become inefficient towards drug protection. Some companies have come up with solutions to overcome this drawback.

1.3 CHILD RESISTANCE

Many prescription drugs still do not enjoy the protection of child resistant packaging. In the early 1970’s when child resistant packs first appeared, blister packs were uncommon. The converse is true today, as now more than 50% of prescription drugs are dispensed in easy open blister packs that represent a real danger of child poisoning. Progress has been made towards developing inexpensive and effective child resistant blister packs, and there is a European standard (EN 14375) against which to test them. As a packaging system for medicines and a range of other products the blister pack is excellent. Indeed, blister packs incorporate a perfect seal, preventing contact either with the atmosphere or with other products and permitting dispensing with no risk of contamination at the retail level. For blister packs, there is however a disagreement as to the definition of an opening. How many units need to be extracted before the pack is considered to be open? In the United States every medicine is accorded a ‘toxicity value’. So for some, a pack is considered open when one unit is extracted, whilst for others up to nine tablets need to be accessed before the pack is regarded as open. Here in the EU, there is not a common regulation for medicine packaging as for non-medicinal chemicals (EC 1272/2008). Therefore, whilst the standard setters have defined an opening for blister packs in EN14375(6) as ‘more than eight units’, the industry has developed products that exceed that standard. Thus far though, big pharma in the EU has only toyed with CR blisters and EU manufactured prescription drugs for the US market are often dispensed in containers with traditional CR closures. The trusted bottle and CR closure manufactured throughout the EU is inexpensive and very effective.

Referred to as reclosable child resistant packaging CR containers comply with an international standard (ISO8317:2003) that tests their resistance to child opening and their ease of opening by adults. Children, 80% of a panel aged 42-51 months, must be unable to open the pack in ten minutes, even after a demonstration; and 90% of a panel of 50-70 year old adults must be abble to open and re-close the pack in one minute.

The Faubel-CRSF solution: The Faubel-CRSF solution is a complex safety element which can be applied like a label on the top face of a blister package under certain conditions. With its material and structure, this solution makes the sensitive top face and the thin strip film saliva-proof, bite- and tear-resistant. This multi-layer solution can be custom-designed on three pages. The adhesive used are certified food safe. Migration is no issue because the alufoil of the blister acts as a kind of barrier. The Faubel-CRSF solution can be adapted to the most varied blister shapes and sizes as well as to specific cavity properties. Three patents actually demonstrate how pioneering our solution is. The safety concept: The Faubel-CRSF labeling solution can be opened without extra tools. It consists of four different “obstacles” based on strength, dexterity, logical thinking (ability to read, to learn and interpret pictograms) and patience (owing to the complexity of this child-proof feature, children will struggle a long time trying to open the package, become frustrated and finally lose interest). Adding a six-barrier protection: To break open this child-resistant solution featuring six barrier levels, you have to be either pretty clever or struggle a long time before getting to the medicine. Of course, any averagely talented person will manage to open this solution within seconds. The barriers can be partly overcome intuitively. Besides, the Faubel solution leaves enough room for additional content in case further details or explanations are needed. After one or two tries, the opening function can switch to an automatic operation as we know it from the pushand- turn-safety closures of cleaning agents, for example. Opening is not exactly a child‘s play, but straightforward enough to allow convenient access to the medicines required in everyday life. Safety is achieved via three mechanical and three logical barrier levels. Mechanical barriers can be overcome with dexterity and/or strength. However, it can be assumed that children in the age group at risk (i.e. between 42 and 51 months in the draft standard) only possess limited fine motor skills and cannot use – or hardly – their physical strength in a systematic way. In contrast senior citizens (according to ISO 8317, healthy persons between 50 and 70 years of age) are very well capable of overcoming obstacles involving fine motor control and of using physical strength in a coordinated manner. The next three levels of the Faubel-CRSF solution are based on logic and can only be overcome by reading and/or by interpreting pictograms, and with experience. Children in the test age group were neither able read nor could they understand pictograms. Besides, prior experiences with the opening of tamper-proof or sealed products could not be assumed. Only senior citizens who were able to read could take part in the test. From experience, adults can decrypt pictograms. As a rule, experience and intuition are helpful when it comes to opening packages. As of January 2010 it is possible to manufacture test samples for blister packagings on a machine especially designed for that purpose.

GLASS REPLACEMENT

A recurring market demand for pharmaceutical packaging and dispensing systems is the search for an alternative to glass packaging, which represents a significant risk of breakage. Some polymers have been developed that do provide a real alternative as these plastics are more resistant to breakage than glass and have high transparency. Furthermore they are lighter that glass and therefore provide a solution that may in some cases have a lower impact on the environment than their glass counterparts. Amongst these synthetic materials, Topas’ COC has been used for some years now for pharmaceutical applications, and glass provider Schott has been offering COC prefilled syringes for more than a decade in addition to their traditional range of glass syringes. Eskiss Packaging is a French company specialized in plastic packaging for the pharmaceutical industry. Their offer include HDPE and PET pill jars; PET bottles, closures and accessories; and glass bottles, glass dropper bottles, and closures and accessories. The focus of Eskiss booth at Pharmapack 2012 was Unicadose, which is a patented polypropylene vial invented and developed by Victor Palusci in 2007. The first patents were filed in 2008, quickly followed by validation of the filling machines. Today there are several million of Unicadose sold in Europe, to the food supplements and drugstore markets.

In 2011, Eskiss Packaging extended its plant of 2000 m2 to reach 4500 m2 and invested in two injection presses and one complete and special line for Unicadose. These investments have allowed the company to double its production capacity that is now able to produce 90 million units per year. Unicadose is available in 10ml and 15ml and a 5ml version is currently under development.

The advantages of Unicadose are : • No more glass fragments • Unbreakable • Nomad • Sterilizable • Clean filling (addition of neutral gas, possibility of conservatives absence) • Hot filling • Color code: no more risks of medical confusion • Easy opening

Unicadose presents environmental advantages over glass vials as the production and the transport of plastics vials produce less than 2.6 of CO2 than glass vials (source : ACV Bio Intelligence Service 2008). Lost plastics trays are recyclable and freight is optimized (reduction weight and space). Furthermore, Unicadose has been designed to be easily transferable on the filling lines of bi-pointed glass vials. This is to enable manufacturers to switch almost instantly from one product to another.

And Unicadose does not limit itself to proven concept of breakable single-dose oral vial, but is already the subject of several derivatives to meet customer expectations: two-phase, with hair applicator, cosmetic, dermatological, ophthalmologic ...

Since January 2012 Top Clean Packaging has become the exclusive partner of Eskiss Packaging to manufacture in clean room the single-dose vial Unicadose. In a press release dated January 2012, it was said that « this vial is likely to upset over the coming months the single-dose world used by the pharmaceutical industry because of its final patient's security, economic and environmental performances. » “The feedback is very positive because the product meets two major AFSSAPS requirements to ensure patient safety in single doses vials: allow easy opening without emitting harmful particles and allow easy and instant content identification. Mitsubishi Gas Chemical (MGC) is one of the companies of the Mitsubishi Group. They have been manufacturing basic chemicals, advanced materials and oxygen absorber for many years.

At Pharmapack 2012, MGC presented their new amorphous polyester for medical plastic containers, whose main features are:

oxygen permeability approximately 2 times lower than PET and 40 times mower than COC ;

water vapor permeability approximately 3 times lower than PET ;

protein adsorbability approximately 4 times lower than COC ;

high transparency ;

compatibility with various sterilization processes, easy processing…

M. Suzuki told us that a line up will soon be available for the production of T-Ester pellets and that MGC are currently developing multilayer containers with greater oxygen and water vapor barrier and low protein adsorbability, as a replacement for glass ampoules, vials and prefillable syringes.

CONCLUSION

Patient’s compliance, child-resistance and glass replacement were dominating trends that VeilleSalon will remember from this year’s edition Pharmapack Europe. Patient’s compliance is a complex issue and a central one for the pharmaceutical industry and health authorities. Patient-related problems can impact dramatically the efficacy of treatments and complex handling can trigger bad administration of the drugs. Inconvenient and difficult to use packages may induce the patient to neglect his or her treatment. Patient’s compliance is therefore driving the need for pharmaceutical companies to have more multi-functional, user-friendly packages that provide the consumer with a variety of functions without being too complex or cumbersome. At Pharmapack 2012, we have seen many companies presenting more convenient and easy-of-use packages for greater efficiency and accuracy of drug delivery. These included pipette with a variably lockable dosing volume, dosing cap for tubes with an integrated quantity regulator, vaginal ovule dispenser, single pill dispenser… We have also seen the emergence of electronic packaging. Smart device containing packaging helps patients monitor their treatment and gives them information like when to take their medication, when they last took their medicine, how much is left in the bottle… Smart devices may also prevent overdosing by preventing the package from being used for a period of time after a pre-defined number of actuations. Child resistant packaging was another major topic at this year’s event, and Burgopak Company received the 2012 Award for their child-resistant slider pack. Some companies now offer desiccant caps aimed at providing greater product protection against gas and moisture. As new desiccant caps delay the activation of the desiccant until the initial opening of the bottle, desiccant compounds stay active and protect the drug for longer. These caps may further include features such as child-resistance or tamper evidence. Finally, glass which has always been the material of choice for pharmaceuticals because of its inertness and transparency is more and more challenged by plastics. High performance polymers such as COC are a real alternative for bottles, vials and syringes when transparency is a prerequisite. Opaque polypropylene on the other hand may also be a material of choice for formulations that do not require high performance material, and where lighter and safer solutions are sought. Just like we have over the past two decades switched from mineral to safer and lighter organic glass for spectacles, plastic alternatives are now making their way towards replacing glass containers for pharmaceutical products.
